Langley, BC - Sucking is a natural instinct for babies and young children, but when they suck on pacifiers or thumbs, it could present a problem for future tooth development. "The actual degree of damage and the age at which it takes place is still a subject of controversy," explains Dr. Aly Kanani, a Langley Invisalign provider. "Experts disagree on several factors." "The accepted wisdom is that pacifier use and thumb sucking don’t usually cause permanent damage until the adult teeth start erupting," notes Kanani, a Vancouver orthodontist. However, a study published in the Journal of the American Dental Association suggests that changes in the length, width, and depth of the dental arch – as well as the development overbites and crossbites – show up in greater numbers in thumb/pacifier users. And the problems sometimes persist after the child has stopped the habit. The researchers suggest that current guidelines discouraging "non-nutritive" sucking may be inadequate since dental changes can start by age two. Thumb sucking is potentially more damaging to tooth alignment than pacifiers because the thumb (or other fingers) is much harder and more unyielding than a pacifier. Between 60 and 90 percent of children ages 1 to 2 suck their thumbs. "Continuous thumb sucking can lead to changes in the shape and size of the dental arch," explains Kanani, a Surrey orthodontics provider, "and can cause the upper front teeth to tip forward." "There are many ways to discourage thumb sucking," observes Kanani, a Langley invisible braces specialist. "Some parents give children rewards for days without thumb sucking. You can also put something that tastes bad (like hot sauce) on the thumb or cover it with a Band-Aid or sock." If none of these techniques work, we can install a "tongue crib," a permanent fixture in the mouth with sharp tongs that poke the thumb when it's inserted in the mouth. That is usually a very effective technique. Although pacifier use is usually less damaging than thumb sucking, it's still a matter of concern if a child is still using a pacifier by age 5. "Most children stop using a pacifier on their own between ages two and four, notes Kanani, a Vancouver braces expert. "However, if a child doesn't do so, a parent might want to step in." Pacifier use is a little easier to stop because a pacifier (unlike a thumb) can be taken away. However, most experts recommend weaning a child gradually from a pacifier and offering rewards to make the process easier.
