Azoospermia is referred to as no sperm count. It occurs in 5% of the infertile male population. Azoospermia is classified into three types. Pretesticular azoospermia, testicular azoospermia, and posttesticualr azoospermia. Azoospermia can result from a problem with sperm production or sperm delivery. The cause includes high fever, undescended testicle, obstruction of seminal ducts, testicular infection, certain hormone disorder. Subsequent diagnosis will focus on determining the cause of azoospermia, which will help health care practitioner choose the best treatment for the disease. The initial diagnosis includes taking medical history, physical examination. Then blood tests are taken that include testosterone and follicle stimulating hormone. After that, two semen samples are needed. With each sample, a standard semen analysis is performed. If no sperm is found, then the semen sample undergoes an additional evaluation in which the sample gets down to concentrate small numbers of sperm at the bottom of the tube. This ejaculation is then examined thoroughly for sperm by an experienced lab technician. If 10 sperm or even 1 sperm is present in the ejaculation, then conditions such as reproductive tract obstruction are painlessly disproved. Pre- and post-testicular azoospermia are frequently correctible, while testicular azoospermia is usually permanent. In the former the cause of the azoospermia needs to be considered and it opens up possibilities to manage this situation directly. Thus men with azoospermia due to hyperprolactinemia may resume sperm production after treatment of hyperprolactinemia or men whose sperm production is suppressed by exogenous androgens are expected to produce sperm after cessation of androgen intake. In situations where the testes are normal but unstimulated gonadotropin therapy can be expected to induce sperm production.
