Understanding Needs – The In-Home Nurse Assessment Once all parties agree on an appropriate level of care, the case begins with a visit by an experienced ComForcare staff nurse who performs a complete assessment of the client’s physical, mental and social needs. Based on the assessment, the nurse then develops a personalized care plan that will be followed by the caregiver at every home care visit. The ComForcare Care Plan addresses both the physical and social needs of every client. This approach allows ComForcare to deliver comprehensive, customized care that is unique to each individual. During the assessment, the nurse will also review the proposed schedule of service and answer any questions the client or family may have.
